Solis Healthcare Privacy Statement

Solis Healthcare is a pharmacy-sponsored organization that develops and administers innovative programs used by community pharmacists in delivering health care to their patients. Solis Healthcare's programs are aimed to assist pharmacies and pharmacists achieve improved medication adherence, medication safety and optimal health outcomes for their patients. For example, our educational programs are designed to help patients better understand and manage their chronic disease by appropriate use of medications.

At Solis, we are keenly aware of the importance of privacy to patients, pharmacies, and to other providers within in our health care system. This Privacy Statement describes how we protect the personal information that we collect, use and hold in connection with our programs.

Our Programs: Health Inform® and Professional Pharmacy ConsultationTM Calls

Two of our programs are the Health Inform and Professional Pharmacy Consultation Programs. These programs are provided to patients using selected prescription drugs. The Health Inform program provides mailings of educational materials to patients about their disease and medication. The Professional Pharmacy Consultation programs, where applicable, ensure patients receive a personal telephone call from either their pharmacist or an automated pharmacy system to remind them to renew overdue prescriptions. We also provide professional pharmacist telephone consultations with patients for educational and follow-up purposes.

The Health Inform and Professional Pharmacy Consultation Programs are underwritten by pharmaceutical manufacturers or through public organizations who pay for the educational materials or calls, as the case may be, and who, along with our pharmacists, may prepare the educational information used in connection with these programs.

Solis operates these programs by receiving a limited amount of personal information from pharmacies about patients who choose to enroll. Solis manages the distribution of information to patients and prompts the pharmacist to place calls to patients.

Participation in these programs is wholly voluntary. Only patients who affirmatively choose to participate in these programs receive the educational materials or the callback service. For patients who choose to participate in these programs, only necessary information is collected (name, address, and details about their prescription) to ensure they receive the appropriate educational materials or consultation services.

Programs may cover only certain prescription drugs or a disease category and a patient must choose to enroll for each specific drug included in the program.

Other Solis Activities

For other Solis programs and activities, we collect certain information from pharmacies about prescription transactions ("transaction information"). However, this transaction information does not contain any information that would permit us to identify an individual person. Prescription transaction information is not linked to personal information that we collect or obtain for our programs described above.

Even though transaction information does not contain identifiable information, we take appropriate steps to protect the security of this information and to guard against the possibility of re-identification.

How We Use the Personal Information We Collect

We use personal information to administer our pharmacy programs. Solis mails educational materials, and prompts pharmacists to contact patients as part of the consultation service. We may also use personal information collected to administer our programs to create aggregate reports that do not identify individuals or contain personal information. Aggregate reports may be disclosed to pharmacies, drug manufacturers or others. We ensure that these reports will only be used as intended and are never shared with pharmacies or others to identify individual patients.

We may also use the personal information we collect for purposes permitted or required by law. We may allow limited research uses of the transaction information that we collect, but only for research projects that have been approved by independent Research Review Boards and that provide adequate assurances that information is protected. We do not use or disclose personal information collected for our programs for research purposes except with a patient's consent, and only for research projects that meet the criteria stated above.

Solis may share personal information with third parties engaged to assist it in carrying out its programs. These service providers are prohibited from using personal information for any purpose other than to provide this assistance and are required to protect personal information transferred to them by Solis in accordance with the general privacy principles described in this Privacy Policy and in accordance with applicable privacy laws.

Disclosure of Personal Information

Solis does not disclose personal information without seeking a patient's consent. Solis reserves the right to disclose personal information to a third party if a law, regulation, search warrant, subpoena or court order legally requires or authorizes it to do so.

Consent

Solis only receives identifiable patient information if the patient agrees to participate in our programs. Any patient can subsequently withdraw his/her participation by notifying the pharmacist who originally filled his/her prescription or by notifying Solis directly by calling 1-866-868-9174. Instructions on how to withdraw participation are included with patient mailings and at participating pharmacies. When a patient discontinues participation in our programs, Solis stops providing the program as quickly as possible.

Retaining Information

We retain personal information about individuals who participate in our programs for as long as necessary to provide the programs and to comply with applicable laws. We securely destroy obsolete personal information.

Security

We use industry standard safeguards to protect the security of personal information in our possession. All of our employees are aware of the importance of maintaining the privacy of personal information.

Getting Access to Personal Information

Any individual can request access to personal information relating to him/her that Solis maintains. You can contact Solis's Chief Privacy Officer at 416-645-1980. Normally, we do not charge a fee for providing access to personal information.

Solis will correct or update your personal information at your request. We want your information to be as accurate and current as possible, so we encourage program participants to tell us when they move or stop taking the medication for which they are enrolled. The simplest means to update personal information is through your pharmacist or by contacting us at 1-866-868-9174.
